چکیده

Trauma is the leading cause of death and disability in the first four decades of life all over the world (1). This causes serious social and economic costs (1, 2). The ultimate goal of the trauma care is to return the patient to an independent and productive life as soon as possible. Trauma surgeon’s role is critical in establishing a better quality of care, because he or she are responsible for taking the patient through all phases of the “trauma disease”, from resuscitation, into the reconstruction and rehabilitation phase (3). Unfortunately, recovery following injury is a complex problem. Combined arterial and skeletal extremity injuries associated with higher risk of the limb loss are the best example of this. This article provides a general overview on the challenges related to the management of the lower limb trauma and specifically looks at the issues surrounding the damage control orthopaedics, limb salvage and early amputation.

LATE ARTERIAL REPAIR FOR LOWER LIMB OR KNEE SALVAGE

The objective of this report is to prove that even after development of ischemic necrosis of the muscles and nerves, we can still save the extremity or the knee and even several days after arterial injury, perform arterial repair and other mentioned procedures. Thus we can retain a functionally satisfactory leg or knee joint, which is far better than classic above-knee amputation and utili...

Delayed amputation in lower limb trauma: an analysis of factors leading to delayed amputation.

An in-depth analysis of the course of events leading to 49 delayed amputation of the lower extremity in 47 patients with open lower limb fractures is presented. Seventeen amputations were performed within one month mainly for vascular reasons. Eleven were between one month and one year, due to persistent sepsis and 21 amputations were performed more than a year after the original injury for inf...

Combat foot and ankle trauma.

Injury to the lower extremity is common in the current conflicts, often severely affecting the foot and ankle. Secondary to continued surgical advances, many lower extremities are able to undergo limb salvage procedures. However, scoring systems still do not reliably predict which patient will be best served with an amputation or limb salvage. Because of this, limb salvage should be attempted w...
